Efeitos positivos e deteriorativos em psicoterapia: um levantamento critico da evidencia empírica

Abstract

The purpose of this work was to analyse the empirical evidence of negative and positive outcomes in psychotherapy, without restricting to any psychotherapic current in particular. In the last years, authors from, different approaches, have been asking themselves just how long outcomes in psychotherapy should remain inquestioriable. One of the major concerns of researchers in this area was to identify and to ascertain outcomes in psychotherapy. Thus, one question was brought forth: is there, nowadays, any empirical evidence that proves, in an unequivocal way, that psychotherapy affects patients for better or for worse? The review by LAMJBERT, BERGIN & COLLINS was critically examined, but rather than criticize each, study separately, it was chosen to specify a number of types of errors and deficiencies which compromise the validity of the data, identifying studies falling in each category. Clinical, theorical and social implications of the mentioned studies were investigated. Because it is an incipient field, with fresh publications and studies sometimes inconclusive, the issues raised should be regarded with caution. Moreover, it must be emphasized that in a single study one can not claim to have included all evidence about the matter.

Este trabalho teve por objetivo analisar a evidência empírica dos efeitos positivos e deteriorativos em psicoterapia, sem cingir-se a nenhuma corrente psicoterápica, de forma particular. Autores das mais variadas tendências, vêm, nos últimos anos, se indagando até que ponto devem permanecer inquestionados os resultados das psicoterapias. Uma das preocupações constantes dos pesquisadores nesta área, vem sendo a de identificar e determinar empiricamente a presença de efeitos em psicoterapia. Em sendo assim, uma questão foi levantada: existirá, atualmente, evidência empírica que comprove, de maneira inequívoca, que a psicoterapia exerce sobre seus clientes, efeitos tanto para melhor quanto para pior? Utilizando-se a revisão de vários especialistas foi realizado um estudo crítico, baseado na orientação de STRUPP, HADLEY & GOMES-SCHNARTZ (1977), em que, ao invés de se tomar cada trabalho separadamente, especificaram-se vários tipos de erros ou deficiências que comprometem a validade dos resultados, identificando as investigações que pertencem a cada categoria. Foram exploradas as implicações clínicas, teóricas e sociais dos estudos em questão. Por se tratar de campo ainda incipiente, com publicações recentes e estudos por vezes inconclusivos, as questões levantadas devem ser encaradas com as naturais reservas, ressaltando-se ainda que, em um único estudo bibliográfico, seria pretensão inalcançável, incluir toda evidência a respeito do assunto.
